Welcome to Blossom Continence Care, where we help you achieve your continence health goals. Our dedicated team of continence nurses provide personalised and comprehensive health consultation services in continence promotion, incontinence management plans and NDIS reports.

At Blossom Continence Care, our dedicated team of continence nurses, Celina and Caroline, specialise in providing personalised continence assessments. We understand the challenges faced by Australians dealing with continence issues and are here to help you regain your confidence and independence.

We support people living with urinary and bowel continence issues, aged 18 years and over. We specialise in assessment and reporting for NDIS plans. A detailed and thorough report ensures that you receive sufficient funding to manage any incontinence issues that result from your disability diagnosis.
If you receive NDIS funding, are a Home Care Package recipient or if you wish to pay privately, we can help.
We do not currently have the capacity to support NDIS participants who are NDIA manged, however watch this space as we will be expanding our reach shortly.
We are based in Sydney, however we can see clients from any state within Australia via Telehealth. For in home visits, we travel to all Northern Sydney Suburbs, Hills District, Parts of Western Sydney and parts of Sydney City and Inner West.

Caroline and Celina will be in touch with you right away and will send our referral form to complete. Once accepted as a client of Blossom Continence Care we will send you the Service Agreement to sign.
Then leave the rest to us; we will set up the appointment with the client or the nominated representatives and take it from there.
You can expect to receive the completed report back within 7 days from the visit date.

We can attend your Continence Assessment in two ways:
While all attempts are made to make an in-home visit possible, for our more regional, remote or interstate clients a Telehealth Assessment is a great solution to bring the health care you need into your home.

The service charge for your continence assessment will be sent to you after the home visit has been attended and upon submission of the completed report. Payment is expected within 7 days. You may be able to reclaim this expense in the following ways:
Please speak with your Support Coordinator or Case Manager about your payment options. For our NDIS clients, please ensure you have sufficient funds prior to booking the assessment. Please note we are unable to bill NDIS/NDIS directly at this stage.
Our Continence Assessments are invoiced as a single flat rate fee that is all inclusive so there are no hidden costs. We charge a maximum of 7 hours per Continence Assessment which includes face to face time, report writing, research and travel. We currently charge at a rate lower than that set out by the NDIS price limits to give you the best value possible.

Caroline is a director of Blossom Continence Care and a Registered Nurse with 18 years of experience. She has coordinated care for NDIS participants and providing face-to-face nursing care where a clinical nurse level of expertise is required. Throughout this time Caroline curated the continence assessment and report writing process to provide best care for clients and to clearly meet NDIS requirements for plan reviews. She has worked as a Clinical Nurse Specialist (CNS Grade 2) in advanced practice roles, focusing on chronic disease management and care coordination and holds a post-graduate certificate in Primary and Community Health Care. In addition to Blossom Continence Care, Caroline also currently works in community nursing to continue to grow professionally and actively contribute to improving community health. Caroline is a subscribed member of Continence Health Australia, the leading organisation for continence health and is committed to lifelong learning around continence promotion and management.
Celina is a director of Blossom Continence Care and a Registered Nurse bringing with her 14 years of nursing experience. Since 2023, Celina has been providing in-home continence assessments and detailed report writing, significantly improving the quality and efficacy of these reports to support participants’ NDIS funding needs. In addition to Blossom Continence Care, she works as a Clinical Nurse Specialist (Grade 2) in advanced practice roles focused on chronic disease management and care coordination. Celina completed a Bachelor of Science and a Master of Nursing at the University of Sydney, followed by extensive postgraduate studies to deepen her clinical expertise and advance her nursing practice. Celina holds a Graduate Certificate in Diabetes Education and Management, as well as Postgraduate Diplomas in Rehabilitation Nursing and Primary Health Care, reflecting her commitment to professional growth and community health promotion. Celina is a subscribed member of Continence Health Australia, the leading organisation for continence health and is committed to lifelong learning around continence promotion and management.
